چکیده

While a number of electrolytes play a role in the genesis of the transmembrane action potential (AP), the changes in the action potential most clearly related to arrhythmias are dependent to a large extent on K+. Potassium gradient is a major determinant of the magnitude of transmembrane resting potential (TRP), and secondarily the rate of rise (dV/dt) of phase 0, and consequently the speed of conduction. The cell membrane conductance for K+, or a decrease therein, is most likely the major determinant of spontaneous slow depolarization during phase 4. Thus K+ has a pronounced effect on both conduction and automaticity. Furthermore, these electrophysiologic properties are altered within levels of K+ encountered in clinical medicine, a situation which, with

Severe metabolic acidosis in a patient with an extreme hyperglycaemic hyperosmolar state: how to manage?

Severe acid-base disorders combined with electrolyte disturbances are of ten seen in patients with hyperosmolar hyperglycaemic state (HHS) or diabetic ketoacidosis (DKA). Acid-base disorders and electrolyte disturbances are tightly connected [1]. The aims of this clinical lesson are (i) to give insight in the relation between hyperglycemia, acid-base disturbances and electrolyte homeostasis, (i...

Association Between Serum Electrolyte Disturbances and Glasgow Coma Scale Score in Patients With Diffuse Axonal Injury

Background: Appropriate clinical measures to prevent secondary brain damage in traumatic brain injury are of critical importance. Objectives: This study aims to investigate the relationships between serum electrolyte disturbances and Glasgow Coma Scale (GCS) score in patients with Diffuse Axonal Injury (DAI) at the time of admission and discharge. Materials & Methods: In this retrospective cr...

Acid-base and electrolyte disturbances in patients with diabetic ketoacidosis.

We undertook the present study to examine the acid-base and electrolyte disturbances in relation to hydration status in patients with diabetic ketoacidosis (DKA). A total of 40 insulin-dependent diabetes mellitus patients (22 male, 18 female), aged 18-61 years with DKA admitted to our hospital during the last 2 years, were studied. The duration of diabetes averaged 9 +/- 2 years. In all cases a...

Acid-base and electrolyte abnormalities in patients with acute leukemia.

Disturbances of acid-base balance and electrolyte abnormalities are commonly seen in patients with acute leukemia. Our study aimed at illuminating the probable pathogenetic mechanisms responsible for these disturbances in patients with acute leukemia admitted to our hospital. We studied 66 patients (24 men and 44 women) aged between 17 and 87 years old on their admission and prior to any therap...

Electrolyte and acid-base disturbances in critically ill patients: a retrospective and propensity-matched study

Background: Electrolyte and acid-base disturbances are common in critically ill patients. Their relationship with hospital mortality has not been clearly clarified. Methods: A retrospective propensity score matched analysis was used to evaluate the effects of electrolyte and acid-base disturbances on hospital mortality in critically ill patients from 2014 to 2015 in our hospital. Multiple logis...

Electrolyte disturbances in patients with chronic heart failure--clinical, evolutive and therapeutic implications.

UNLABELLED Patients with heart failure are, by definition, hemodynamic unstable. Often, this condition may be accentuated by medication (digitalis, diuretics, antiarrhythmics), so that they become more sensitive to electrolyte disturbances, reacting sometimes violently with severe and life threatening rhythm troubles. AIM the evaluation of the incidence of electrolyte disturbances in patients...
